Please use this form to nominate a patient or patient family member for the NHO Patient & Family Advisory Council. Ideal patients will meet the following criteria:
- Recent Experience as a patient or family member of a patient at NHO
- Ability to represent their patient care experience
- Willingness to work collaboratively and in an advisory role
- Good listening skills
- Ability to interact well with different groups of people and with differing opinions
- Respect for others’ perspectives
- Positive, constructive attitude
- Ability to maintain confidentiality
- Ability to participate in a consistent and agreed-upon schedule of meetings
- Commitment to serve for a one-year term with the option to renew at the end of that term
