Your First Visit

Knowing what to expect and what to take with you to your first appointment can help make things run more smoothly. Because you will be receiving a lot of information, it is a good idea to bring a family member or a friend for support. At your first appointment, you will meet your physician and cancer care team and learn about your diagnosis and treatment options.

After your first appointment is scheduled, one of our Patient Coordinators will be in contact with you. You may complete our required new patient forms electronically by clicking the buttons below.

HEALTH HISTORY QUESTIONNAIRE

AUTHORIZATION TO RELEASE HEALTH INFORMATION

Your past history and family history are important for your care. It is extremely important that you complete this form carefully as this will assist us in your care. 

In addition, we will need you to fill out the Authorization to Release Health Information form. 

1) Complete these forms in their entirety

2) Hit Submit on each and your information will go to our Patient Coordinators

Bring Your Driver's License, Pharmacy, and Insurance Cards

We will need to obtain a photocopy of all of these for your file.

Bring Your Medications List

Please provide information about any medications you are taking. Include any herbs or vitamins as well. Be sure to know the doses and how often you take them. It may be helpful to bring your medications with you on your first visit.

Bring Copies of Your Advance Directives

It is important that your physician understands your choices for health care. CLICK HERE for helpful information about making those decisions. If you have advance directives already prepared, please be sure to provide us with a copy.
